Controls for configuring display of threshold lines on quantitative annotation

Provide GUI controls to add, remove, and change the value (and appearance) of one or a group of annotation rows. This was previously possible due to an "emergent feature" of the colour by annotation dialog, which has now been addressed (Cancel restores state of all annotation rows touched by the dialog, where as before, thresholds were left at their last adjusted value).
